Insurance for Injury Treatment Massage
For injury treatment massage, I accept most major private health insurance plans including Premera and Regence, along with Personal Injury Protection (PIP) and Workers’ Compensation claims (often referred to as L&I). Direct billing is provided for covered PIP, L&I and private health insurance, and super bills are available for client submission to out-of-network private health insurance companies.
Injury Treatment Massage Prescription Referrals
All insurance companies require a prescription for massage. Please be sure to have that prescription ready on your first visit. If you are seeking injury treatment massage and do not have a provider to write that prescription for you, please contact me and I will share a list of possible providers with you.
New Patient Forms for All Treatments
Whether you are seeking injury treatment massage, relaxation massage, or shamanic healing, there is a set of forms to be completed and added to your patient file. The Fine Print
Time is precious to everyone. With that in mind, we respectfully request a 24-hour notice for cancellations. Additionally, if you are running more than 15 minutes behind your scheduled appointment time, your appointment will need to be rescheduled. And finally, while the hustle and bustle of life can sometimes get in the way, repeated cancellations or tardiness which undermine treatment may require a termination of the therapist/patient relationship.
